Not really, this is worse...
two Vanuses appear because there are two quests corresponding to him in the mage hall, and since NPCs in ESO are "created" by the quests you are having now, 2 of NPC appeared
Chime-Maker case is just plain bug out.
There is also a weird time that my quest maker mistaken one of other player's follower (due to quest) as the one I should talk to, thus I remember 3 or 4 of us chasing another player, all confused...2 -
