Resurrecting on Velidreth bugged

Last night we could not resurrect on Velidreth hardmode about half the time. Either the prompt never appeared for certain players (in one instance another player could still be resurrected at the same time) or resurrection completed but nothing happened for the dead player. I observed it a few times when someone died in the maze, but it also happened on death to the spikes.
