vHoF terminal feedback hitting harder than it should

Assembly General, we were killing 2 terminals, but we were getting hit for 34k + dmg by terminal feedback, as if 3 terminals were still alive.
After the first couple times we made absolutely sure the first terminal was dead before moving to the next, so, yeah, the first died, positive.
It wasn't happening all the times, but it was happening often enough to be concerning (4 consecutive wipes because of it). We eventually decided to use Bolstering Darkness, but I'm not sure we get past it because of it or it was just one of the times it worked as intended.
I'm asking the others if anyone recorded it, will edit this post with a video if we have one.
