Gravity Crush ultimate doesn't appear but ultimate gets used

For the past week, on my templar, every other time I use my Gravity Crush ultimate the points get used up but the actual attack doesn't appear.
I thought it might be me switching bars too fast, but I tested it out and even without switching bars it just doesn't appear sometimes.
