Visual Outfit Weapon Bug

Platform: Xbox
Zone: Cyrodiil
When I am fighting and there is lag my outfitted weapons swap bars.
I use a destro (EP Style) and s/b (EP Style) on a DK
During / After the fight as I change action bars I will end up visually wearing a shield using destro spells and vice versa.
Solution: I need to re-zone or use a wayshrine to another keep to reset them.
Apologies if this has already been reported.

Please please please address this, it is such a painful bug to deal with. You never know what bar you are on and you have to constantly look at the skill bar to see what weapon you are using. Every 5 mins it bugs and then becomes normal again, it's so confusing.
I'm on PC EU, this happens to me in every zone of the game, not only pvp related
