Vision quest, stuck at Library trap door

The Vision quest is bugged for one of my characters and I am stuck. I am at the trap door with the items I had to collect, but Tedras Relvi won't apprear to open the trap door. I abandoned the quest (twice), disabled all add-ons, reloaded UI, restarted PC... went on vacation... but same issue every time.
I fulfilled all the objectives and my new objective is : talk to Tedras Relvi. I went to the library trap door, the portal that should make him appear shows up for a second then disappears. I tried relogging, UI, vacation - rinse and repeat - but no luck. He is not there.
