Werewolf not working PS4

When I hit my werewolf ultimate it always bugs out. Either it uses my ultimate points and doesn’t do anything, or I transform for 1 to 5 seconds and get kicked out - but can’t swap bars until I hit the ultimate again (as if I am still in werewolf form)
