Is that the guy where the grates open up and spew poison? If you listen to the NPC, he will tell you what to kill and bring him and throws down a power boost after he makes a concoction.
Now the end boss, IMO, needs a tweak. The statues that shoot out disease and instant kill anyone (even in magma armor) doesn't give enough warning. The red isn't "get out of this", it's "hey, you are dead." You can remedy this by watching statues, but at this point, the red serves no purpose vs how it does in rest of game.
