It is definitely an issue with external displays. There is a workaround -- switch desktops back and forth to reset your cursor -- but it is so COMPLETELY annoying. For example, if you want to swap abilities, when you bring up the abilities window you get a bad cursor, and when you place the first ability the cursor goes away. So you have to switch desktops back and forth to move the next ability, and so on.
It is frustrating that nobody has even acknowledged this issue.

As cool as it looks I can't actually see the pointer when trying to click on potions or gear and have to move it around until I see something get highlighted. It also can't drag and drop without leaving an imprint of the gear on the screen permanently. Alt - F out of the screen will return it to normal until I need to use it again.
Any tips? I can't screen shot it because I have to use the mouse and it makes it disappear.
Have you tried:
System Preferences > Accessibility > Mouse & Trackpad > Uncheck Spring-Loading delay

Thank you so much this fixed the issue on my iMac (27-inch, Late 2013). I really appreciate it.My wife had the same issue with High Sierra - the fix I found on a forum for a different game. Go to accessibility settings and increase the cursor size.
