direct damage attacks not registering or can't target right

Anyone else been having problems with these. On my tank, pierce armor seems like not hitting targets right. I have to aim way high to make it kind of work.
On my StamBlade killers blade, poison injection, surprise attack and twin slashes are acting all messed up.
It was bugged like this before Morrowind and didn't get fixed til Morrowind dropped. I'm assume it bug back again.
