Fossilize Perma CC

Since the Horns of Reach update on XB1 NA with my MagDK. At random after a Fossilize on an enemy near death they get stuck in a Perma CC state and can not switch bars, cast and abilities, and can infinitely CC break animation.
I have fought other MagDKs and not had this happen to me though or noticed it happen in any other MagDK fight. So I think it may have something to do with the sets im running?
Its very annoying for the person that it happens to because the only way to fix it is for them to log out and back in.
