Keep getting UI error codes.

I keep getting UI error codes and have no idea what they mean. The one I get the most says something about duplicate key binding and when that happens I can't use the X or circle buttons. This also prevents me from making an in game report or signing out. Any idea on what's going on and how I can fix this issue?
