All these bugged achievements

After One Tamriel many achievements are seriously bugged. The worst situation is with dungeons achievements. Example:
I haven't got achievement for completing veteran Darkshade I, even if I killed all the bosses:
As you see, looks like last boss Sentinel of Rkugamz was not killed.
But at the same dungeon run I got achievement for killing last boss with hard mode (which is obviously impossible to get if last boss is not killed):
Also I got no death achievement from the same run (which also requires last boss) even when all group members were dying in this run:
This is only one example. For "Vaults of Madness Conqueror" I needed to kill second boss dozens times to finally get this achievement... Also I was getting "Trial Damage Dealer" achievements on alts who never were in a trial, "Dungeon Ruler" while farming dolmens etc... It's bugged since months and it prevents me from faster levelling up undaunted skill line on my alts.
Fix this finally please, it's since 5 months.
