Anyone else having problems with synergy?

I've had a few cases of when I press triangle and circle to do a synergy it casts my ultimate which is super annoying and a waste since it will cast it way far away where my cursor is pointing.
It's mostly been in ICP during the slimeball boss to kill the zombies but also had it happen in direfrost trying to synergize a skill.
Any idea if it's ESO problem or my controller PS4 crapping out on me?
