Keep Getting Logged Out Mid-Duel/Mid-Dungeon

Boyfriend and I both get logged out mid-duel or mid-dungeon or upon login. Lot's of DCs since One Tamriel. It usually goes like this:

Log in, run around, screen freezes, logged out.

OR

Infinite loading screen into dungeon, fighting as usual, everything freezes, logged out.

I usually get randomly logged out too. Of course it always seems to be in the middle of an overworld boss.
