Auto-Equipping Off-Hand Looted Weapons/Shields Glitch

Whenever I loot a Shield or an Off-Hand equipable weapon, the game automatically equips it, replacing my Two-handed weapon...regardless of item quality.
For instance I'll be killing NPC's with Blade of Woe, when I loot them, it will replace my main-hand Legendary Bow, with an off-hand white CP 150 Shield, dagger, mace etc.
This has been happening for the past few months, primarily when I loot stolen weapons from a dead NPC.
Is there a way to avoid this?
