Quick slot bug?

Every now and again, I am experiencing a bug where I cannot quick slot items. For long periods, there is no problem, then suddenly I will find that after I have placed an item in a quick slot it does not show up in combat, whatever I do. Repeatedly putting it back in the slot makes no difference and the bug has been around for weeks, if not months. Anyone else getting this??
