Name of group frame addon

I have seen several people with such group frame several times, some of them used it for more than half a year. What the name of it, is it some outdated addon with such special effect?

Many addons let you add resource numbers to the original ui, instead of replacing it.
For instance using LUI extended I can choose, for player/target/group frame (each one independently from the others), if I want to do nothing/add resource numbers/completely replace with the custom one.
I am not saying that the one in your picture is LUI extended, but the outcome is very similar.

I like to use healers group frame, it displays hp left, max hp, shield, if there is a hot active on a player, which class he is etc.
The main reason I use it, is for the bigger hp bar and that you can rerange groupmembers, which is quite nice for organizing groups for AA or other trials
