Just curious how players got "the one and only" title?

Some addons include code that change the titles for specific characters (i.e. friends of the addon authors). If you have one of those addons installed then you will see the modified titles on only those characters listed in the addon code. If you don't have one of those addons you see the normal titles. There is no addon that allows you to create and apply your own titles.

OP. I don't have an answer as to which acheivement unlocks the title.

Some addons do substitute the text for the Volunteer title. Skyshards is one, off the top of my head. It's not user configurable. As someone reminded me, this is set up and configured by the addon author (and for awhile was available by request, as I recall), but it's not something you can set up yourself.

Surprised this would be visable to others. Someone can gives themselves the hardest title to get in the game without ever dealing with the content.
