Disappearing NPC Followers

Recently I have experienced a disappearance of quest oriented followers vanishing without a trace, a couple weren't too bad while some made it very difficult to complete the quest or unable at all when I'm told to talk to the NPC who is no longer there. I recently did Valley of Blades main quest where you normally have Lyris, Sai, and Abnur by your side; however, after going down into the crypt talk to Sai Sahan first, go down grab the amulet... they all vanished. I ended up fighting all the Undead champions by my lonesome, nearly died if it weren't for Werewolf form and my friendly Dire Wolves.
The most recent one was trying to do the Public Dungeon in Deshaan, my NPC follower disappeared halfway through the dungeon unable to open the door to the safe zone as she's required to interact with the door. I didn't die along the way and when I went back to the start of the dungeon she was nowhere to be found.
