Problems with Blessed Crucible and Blackheart Haven

I have run and completed both over a dozen times, yet I haven't been given the Completion for either. I have run them as Daily Pledges and not.

Anyone else having this issue?

(Yes I have killed all bosses in both each time.)

and you can add Normal City of Ash to that list. Now completed four times. No achievement/completion in the Journal.
