Query on Game Client and Login Servers

Is there an issue with the Client login system?
1: when ever there is a patch and i login AFTER patching " you are not authorized please submit code"
2: when i give the code I ALWAYS have to do it twice as first time states: "unknown error"
3: Why is my Ping to all other games 30-50 ms but in ESO its 80-120? and in wrothgar the ping goes up higher... which seems more server based than client based
