Self revive bug?

So lately I have been getting quite an annoying bug, where i cannot revive myself with a soul gem on the death screen. mashing E as much as I can and nothing happens. So I'm often having to rely on party members to revive me or having to go to a wayshrine and trek all the way back to where i last was.

Anyone else getting this? anyone know a possible fix?
