From my experience it is triggered off of light/heavy attacks and skills from weapon skill trees. This also seems to include damage from dots such as the one from destructive reach, and poison injection if I recall. With enough dots and weapon skill hits the heals proc regularly, check for a green glow on your character to see when it triggers.
I used to run degeneration with my magicka sorc. I really enjoyed the self-healing capabilities with just Degen + crit surge. Light and Heavy attacks proc degen health restoration, as well as weapon attacks like Force Pulse. I'm not 100% if this is still the case, but since each individual elemental damage type with force pulls is counted individually, each one can heal you. If one element crits, so do the other 2, which means 3 crit heals from a single cast.
I was pondering the usefulness of using it with Pulsar in a Mob. With high DPS and a 15% chance per individual in the mob triggering the effect, seemed like an effective way to survive while burning down mobs.

I think it also procs of overload light/heavy attacks.
Weapon attacks are just the LMB attacks, otherwise its phrased as weapon abilities or something along those lines.
It does proc on overload last I checked, and again as far as I can tell the skill saying "weapon attacks" is referencing light/heavy attacks and skills.
