Hold the light attack just enough so that two fire balls shoot out instead of just one, and follow up with a crushing shock right after that.

Lefty Lucy has a nice video on it here:
    https://youtube.com/watch?v=eTb2YpuRsP4

The short form, in case you can't watch the video, is that for light weaving you trigger a light attack and as soon as the animation begins to play you hit a skill to cancel the light attack animation and essentially fire the light attack with less cast time than it should normally take.

For medium weaving you do the same thing but keep the attack button held a moment longer (but not until the heavy attack fires). The idea is you want to keep hold the attack long enough to count as a Heavy attack but not pay the animation time than a heavy attack is supposed to require.

That's because different things have different animation priorities. Lefty covers this in the video without actually saying it outright, but if you can keep the priority order in mind you'll always know which will work.

    Bash/Block > Skills > Light/Heavy attacks

You can also animation cancel with a weapon swap, but I that's tougher and I've never got the hang of it. If you are on PC, running a combat long and checking it after combat will let you know how successfully you are animation cancelling during a real fight.
