This is what is classically termed in sports as "choking" and when it came down to it, when he was in the final play of the game down 4 with 4th and goal he choked instead of pulling through an unforseen circumstance.
That's no insult to Sypher or excuse for Zenimax, just something happens to us all, even the best of us when we set our focus too firmly on the end goal that we want so badly and end up losing sight of here and now. I bet if Sypher ready had his flawless that he would have pulled that situation off 9 times out of 10.
Any sorc that uses overload deals with this bug a dozen times a day. I consider it a "feature" at this point. The fact that his animation went invisible didn't actually hurt him. He wouldn't have killed the boss regardless because he kept attacking instead of bashing the channel. Perhaps he would have bashed it if he hadn't had bugged animations and tried to squeeze out the last bit of damage. I definitely panicked/chocked after not killing the boss. Because I just didn't see him surviving as a possibility and it took me by surprise (and it wasn't a possibility if it weren't for the bugs).
Overload Heavy Attacks were ticking for 8K and my light attacks ticked for close to 25k. My Overload got stuck on heavy and because I was unable to see my animations I didn't realize it before it was too late. By that time, I panicked, and got killed as a result.
I don't typically use overload in pvp so I'm not familiarized with the inherent bugs it has. It was just unfortunate that both the bugs happened at the same time. Because if only of the bugs happened I would have killed the boss.
If the heavy attack bug happened I would have seen my character doing the heavy attack animation, I would have quickly cancelled it and then began light attacking.
If the no-visual bug happened I would have still thrown light attacks and killed the boss even though there was no visual.
The unfortunate thing is they both happened, causing me not to realize I was casting heavies and as a result not having the dps to kill the boss when he was definitely low enough to be killed.
Ahh, I didn't notice the hits were heavies. Yeah if your overload hadn't been stuck in heavy you likely would have killed him...although for whatever reason he never dies at 0% health for me. It always takes a few more hits to kill him so who knows for sure.

I almost had it as bad as you before I got my flawless. It was the third or 4th round when a daedroth spawned and I had to cas t my ghost explosion to stun him. A ghost got too close and I dodge rolled away from it then hit my explosion synergy....only to pop a damage sigil instead. The daedroth was already on top of me breathing fire by then and even though I hit X again to use the explosion it was too late and killed me regardless.
Why they decided to make the explosion synergy the same as picking up a sigil is something that I'd put money was on was a troll attempt because from an interface design standpoint it was a terrible decision.
