Forgot game login credentials. Was able to log in through forum.....

I have been seeing a lot of good things about the game lately and I heard that it has completely changed since launch, so I tried to log in on the website just now to download the game and start playing again, but what I thought was my login information no longer works. I tried to do reset user id or password, but they ask for First and Last name on the account and I never use my full name, I usually only put my initials in or half of my name. All of the name combinations I have tried doesn't work either, so I am stuck. As you can see, I was able to log in to this forum account and on this account it says "Joined Feb 2014" which is correct, but the same credentials cannot be used to login to the actual game account. How do I get my game account back?

Also, when logging into this forum account, I noticed my name was changed to "haoleflipub17_ESO" but I don't remember making my username that. I mean, the "haoleflip" part is correct, but the pub17_ESO part had to have been added. Could this be part of the reason why I can't log in to the actual game account?

1 you me account is completely different from your forum log in
2 that name was assigned during a public beta 17 (added onto your name)
3 you would need to create a ticket to recover your actual account here- https://help.elderscrollsonline.com/app/home

On the right of that page to create a help ticket instead of resetting password
