Vile Manse Quest Bugged?

My friend and I were running this group dungeon. We did the quest for it, and killed Graccus boss. I got quest credit, but she didn't. The altar wouldn't activate, etc. We've tried soloing it, redoing it, waiting months on patches to fix it, etc. We're stumped. Is there a fix?

TrueGTI✭I just ran the quest last night on Xbox NA had no issues. Sorry about the problem I hate bugs like that.
