[PS4] Damaged Equipment disappearing?

I was deconstructing outdated gear and among it was an armor piece that reached zero durability. When I went to deconstruct the equipment I noticed I only deconstructed 6. I might've deconstructed the damaged piece without realizing it but it sure didn't seem to show up. Can anyone confirm if this issue has happened to you as well? The issue being unequipping fully damaged gear makes it vanish...
