I decided it's time to post my build here on the forums to hopefully help others as others have who have helped me. I welcome any constructive criticism as well as comments and questions below.
Let me just start by saying this build is not for the faint of heart, meaning if you're the type who likes to play a specific roll this will be very challenging for you to adapt to play.
The "Berserker" Templar is as stated above, is an extremely versatile build that allows a player to Tank, Heal and OR DPS all at once (depending on your individual skill/dexterity) or play any of these rolls individually with relative ease. As I'm posting this I only have 122 Champion Points and still using mostly V14 gear so this build is pretty easily attained for most players.
For this build I'm an Altmer Templar utilizing dual swords as well as sword and shield just based upon scenario. Obviously DW is preferred for DPS or Healing since it currently grants more spell power which means better heals or spells.

As far as gear goes I'm running mostly V14 stuff for all my armor and weapons except for one sword. Currently my gear is: 4 piece - Martial Knowledge ( Light Armor: shoulders, belt, gloves, head), 2 piece - Seducer (Heavy Armor: chest & legs), 4 piece - Healer (Light Armor: shoes, Jewelry: neck & 2 rings) and 2 piece - Torugs Pact (Swords and Shields, I have 2 separate Shields and a total of 4 Swords to suit my combat needs at any given moment)
All of my armor has Divines trait (Except the Hat of Martial Knowledge) and I am currently running the Atronach boon for increased Magicka recovery.
As far as Champion Points go, as I said I only have 122 and I have distributed them as follows: 10 points into Elfborn, 15 points into Spell Erosion, 15 points into Thurmaturge, 9 points into Spell Shield, 32 points into Light Armor Focus, 14 points into Magician and lastly 27 points into Arcanist. These are the only areas I will continue to spend Champion Points as I accumulate them also.
Here are the skills I use based on what I need to accomplish. The addon displayed here is called Dressing Room and I HIGHLY recommend it. Below I will post pictures of all my unlocked skills and passives so you'll know where to put points. Passives are EXTREMELY important to all builds, unlock as many as you can that will benefit your playstyle.
Keep in mind for passives I'm missing points in that if they were leveled I would put the missing points there to further boost my stats.
