Disappearing Inventory Items

Three x's in the last 10 days I have had items go missing from my character and or inventory.
I time it was a helm my toon was wearing. I did all the normal checking of buy-back and my bank as well as /reloadui and re-logging. It never was found.
And last night it was a stack of 29 runes i pulled from the guild vault. Banking history shows me pulling the item(s) but it never came to my inventory. I also check my bank and it didn't go there.
Lastly a Alchemy writ reward disappeared after 'using' it. The survey just vanished from my inventory.

Have you tried removing your addons (or even updating them) to see what happens?

Are you using any add-ons? Some add-ons may filter what's in your inventory and not show some items. I've never had this happen to me in 1 year+ of playing. As for the survey, they disappear automatically when you find the corresponding location. Could you have opened it at the survey spot and have had it immediately spawn the nodes?

I have....even did it 1 by 1 thinking that was the glitch. Really tried anything I thought at all that could be related.

When i opened the little alchemy prize from doing the writ it had the survey and a few other items like normal. I opened my provisioning and blacksmithing writ prizes as well. But when i went to open that survey it just wasn't there.. Everyone before and since has worked fine.

logged on the other day and was wearing totally different head and feet then I logged off with luckily my set pieces were in my inventory. But never changed to that armor so something weird happening.

The PC Games been doing that for a while. Emails are also being affected. Sending things to people they never get and are never returned to you.

I had a weird glitch related to inventory.

In my inventory I had:
Steel Dagger of Frost (Green, Picked up Item).
Steel Dagger (White self crafted item).

I was going to deconstruct both. In deconstruct menu, Steel Dagger of Frost was listed first, then Steel Dagger. Right next to each other.

I deconstructed the Steel Dagger of Frost, it was removed from the Deconstruct list.
My white Dagger then changed the name in the deconstruct list to "Steel Dagger of Frost" (still white). When selected it showed the item correctly (meaning no enchantment) but the name was still listed as Steel Dagger of Frost. Both showed it as white.

I backed out of crafting table; re-entered Deconstruct and it showed Steel Dagger again.

Now, in the long term, this was meaningless; however, it worries me that such an inconsistency exists, even temporarily. In my experience, temporary state consistency issues usually indicate sloppy code and often a failure to scope the memory allocations correctly; although I will admit, rarely, it is a behavioral problem with a method interacting with the buffers.
