In Bankorai, during the quest - The Lion's Den - , when attempting to go into the at-Nimr estate my character appeared behind the door, but zoned into the building. I turned around and fell underneath the world and then respawned at the Hallin's Stand Wayshrine.
This was during the part where I had to follow Qimah and talk to her within the house. The only noteworthy thing I did was I didn't actually follow her, and instead went to the quest marker (her house) and went in before she was there.
