Based on a few real life friends and even non-gamers who signed up on day 1 when the console page opened. All of them who also did the email survey, got a beta key.
I don't think it's too random but perhaps based on basic things like Age, if the name you used seemed real and if you entered your email address correctly.
Now if you signed up later...no idea but usually those who really are watching for acces don't mess around when sign ups open

I signed up twice. Once long before the pc version came out. And again when the transfer offer was announced - I received an email regarding what system I would like to apply for, and I did so in a timely manner.

I don't know but I don't think it's fair to those that have been waiting for it to release on the console, whilst supporting it on PC the past year.

Haven't gotten mine yet either. Subscribed all last year, paid $20 for the upgrade, signed up for the console beta last June, and filled out the survey on time.
