Use the Priest of Arkay in the mages guild. Ebonheart Pact - Riften , Daggerfall Covenant - Evermore and Aldmeri Dominion

Reapers March , they are in the towns where you completed the vamp. quest.

Take a look at this guide - http://teso.mmorpg-life.com/become-vampire-eso/

If you scroll down the page you'll find Cure section with detailed instruction and screenshots where to find NPC.

I hope the guide will help you.

Read the guide carefully. I seem to recall something about doing a respec BEFORE the cure to release those skill points. If you do it after, the skills get locked (?). I don't remember exactly, but probably worth checking first.

The way it used to work was that any skill points spent in the vampire line would remain there after you cured your vampirism.

If you became re-infected, you'd find that the skill points were still in the vampire line. So you didn't technically lose them, they were just sitting in the vampire tree.

Not sure if this has been changed since the patch.

I thought I read that once cured, you can never be a vamp again.

It still works that way. Your skill points are "saved" in the skill line until you do a full respec. It doesn't matter when you do the respec though. You can become a vampire again,
I have approximate knowledge of many things.
