What's the difference between the Pts and live?

Why is it that we have a test server that people supposedly use to test features and functionality of new stuff, but once it hits live it causes problems.
How can bugs and breaks in the Pts get passed into live?
Who signs off on that update to be made live? What's the process?

I've seen answers to the above ranging from "we can't check everything," to "" We are currently working on a fix for that"

Is there something different between the two that things work better in pts than live?
I'm at a loss here, maybe someone can explain how these things happen.
In short why have a pts if it's going to be released on the live server with problems?

    Coz they want to get rid of the main visible bugs so it can be played poorly. Then patch it up as they go while giving people something to play with and occupy them so they dont leave. Its basically an effort to keep people playing even though the game isnt ready because people are impatient and ZOS is scared of losing players.

    Been saying this for months, Why have a PTS if you don't use it change/update/fix issues rushing half baked patches see 1.3 just kills the pop.

    for one not all reported bugs get fixed before the patch goes lives, depending on the impact of the bug and the amount of people it happens to this is normal for mmos and not all too bad.

    way worse and not understandable is that zos keeps making changes which never got tested (by players)

    last pts version was 1.6.4 going to live was 1.6.5 with some new juicy bugs included -.-

    You here almost a year. It's been here from the beginning: you go on PTS, submit tons of tickets about bugs and then all these bugs goes on Live server. That happens every update. IMO they need PTS just to be sure that new update won't crash in first 15 minutes.

    I have no idea . It's not even a megaserver so big pvp tests are impossible .
