I was thinking, wouldn't it be great to have a timer so if you don't complete the main quest in time, your characters features begin to have that melted waxen appearance that you see on the Soul Shriven at the start of the game? And the longer you leave it, the worse it becomes; however, once you get your soul back, you are restored to your original appearance.
After all, as you were told by that Argonian Soul Shriven who released you from your cell "It will; You are new here yes? Over time your appearance will change"
This is only a visual thing and has no effect on your stats, so you don't need to worry about skills deteriorating.
What do you think? It would certainly make people want to complete the game, but we should be lenient and make the effects start to take place after, say....four months, and after that it changes more once two months pass, and then one month, and then every week until you are at the worst state possible.

Later The Prophet will restore your link to Nirn (before actual escape), so you can exit Coldharbour. It means you won't become feral.
