What is considered a "Low Health" enemy?

Some tooltips in-game proc certain things on "low health enemies" but don't state what percentage "low health" begins. The one I'm questioning is Disintegrate. I've heard multiple Sorcerers say they use a Shock enchant on their staff to proc Disintegrate before the 20% execute phase, in which, Mage's Wrath (or either morph) is the sole ability until the boss is dead. I'm curious if anyone has been able to successfully test the percentage with accurate results.

tl:dr What health percentage does an enemy's health have to reach before Disintegrate can proc?

I believe Distegrate can proc on anything under 25% health (24% or below)
