I've been PvPing today and seen several people use both the Dragon Knight and the Nighblade abilities on the same character. I've even got a screenshot of my death recap of the same person using Swallow Soul (Nighblade), Burning Talons and Molten Whip (Dragonknight) Abilities.
Is this possible to do or is it an Exploit?

    That's because he reflected your swallow soul.

    I saw a player once who was both a vampire and a wear-wolf. I didn't believe it at first then to prove it, they turned into a WW and used bat swarm at the same time.

    No, not all. Things like NB's Cripple and one of the execute morphs of sorcs (iirc the other one will soon be fixed too) aren't considered projectiles. There may be more but I can't remember all of them.

    Yeah with a combination of Annulment pre nerf and Ward, it's possible.

    I honestly could have sworn I was fighting a DK + NB the other day. Nothing in the death recap since he didn't kill me, but he was using a sword and shield and I was Ambushed (noticeable animation compared to the alternatives which would be shield charge) and then Taloned and Burning Whipped. There was also particle effects around him (not sure what abilities) that I've never seen a DK have before.

    Or magma shell synergy even without any combination.

    I don't know about that.

    I myself think it should only reflect spells, but I usually just get out a giant greatsword or battleaxe and charge them, so im a little bit biased about it.
