Why do NPCs I'm talking to generally look over my shoulder while speaking to me?

Yeah, I know their eyes can dart about, and they do actually look in the general direction of my eyes on occasion, but seriously, they spend a lot of time looking over my shoulder.
And it's not like I can be standing someplace other than exactly where the devs know I'm going to be standing while interacting with an NPC.

Unless the speaker is showing me something, or there's an incident we both turn to look at, or I'm actually reading something on the screen when the speaker walks in and starts talking to me......?
Unless I'm blinking or sneezing, I'm looking at their eyes.
Because that's what I do when I'm paying attention to someone.
