Anyone seen this before? I havent.

i wonder if this is where we see "use left mouse button to exploit" and maybe some one did do it that contributed to your final death blows?
i dont think it's a hack, i think it's part of the synergy attack

One of the flame lash morphs can be used a second time to deal extra damage and heal the dk
