They really should have thought of this and made it so that the journal for this favour was different than the one for the original quest. Maybe that's something they can fix.

I'm curious though, why save a quest item in the bank that can't be used for anything else ever again?

I guess, I found it one day with one of my toons, and didn't know where it come from or what it was for, and just secure it in my bank for no reason. Not the most clever action, obviously, but well...

That makes perfect sense why you kept it. Not knowing if it was something that may be needed in the future.

What is strange is that a quest item didn't just disappear once the quest was done. Because this is now causing problems for players to complete the favour.

It does. But it doesn't disappear before you've read it and thereby started quest. The problem is that it is not read automatically, you need to interact with it from your inventory to start quest, and it happens to be not so obvious for players.

Oh, so it starts a quest. I thought it completed a previous quest. Either way it's now causing confusion.

Thanks, I found Nedras Book in my bank account! It have been there for years, I guess....
So, I could finish the quest. Thanks a lot.

Good to know there was a workaround.

As a concept, I like the idea of going back and catching some of the off-beat quests and/or returning to nearly forgotten locations to complete Favors. In practice, some older content doesn't always behave consistently, and it's impossible to test twenty such quests in a row on the PTS. Inevitably, there will be bugs that make their way to the live servers. Fortunately, the worst of them was patched and others seem to be able to be overcome.
