have youtried turning all your addons off and libs and restart game and try playing it like that for a while and see if its any better and then might know if its an addon issue
I have the same issue, while my FPS shows 120 FPS, gameplay is NOT smooth, like not even a bit. It's so slow/tearing idk how to tell but it says 120 FPS but I KNOW it isn't. It's just hurting my eyes.
I had this issue even before Update 46 when my machine updated to Windows 11 24H2. As soon as I reverted it to 23H2, it was fine. Every so often Windows forces the update on me and I have to revert it again because I immediately notice how bad it feels.

I have only had fps issues after the update.
I had around 100fps everywhere before the update. After update im on 80-100fps but have sudden drops to 20fps random places in pvp and where i had 50fps before in pvp i now drop to 10-15fps slideshow....
I can play Battlefield 1/5/2042 on full graphics. Eso i have tried high, low, with addons and without addons, no difference after update.... I have also tried editing the usersettings.txt file to no luck. My pc is just a few months old with a rtx 4060, i5 12400 and 16gb ddr4 ram.....
Guess ill just leave the game and take my money elsewhere....
If you have stable FPS it could be connection related. I have this problem, but it's because my ethernet adapters are getting old.
Try running Windows task manager and run around Wayrest/Elden Root for a couple of seconds. Then alt-tab to task manager and see if any of the components under "performance" are hitting 100%.
That i have done. I have run 4 screens for years. One for the game and others for discord aso.
I have task manager up when in game and it shows prosessor go 100% when the fps dips. And eso is the only thing using high amounts.
Right now im installing the game fresh....
I'm not sure how or why, but after the June 25th maintenance even FPS, which is normally a video card on the client end issue, but even FPS was radically impacted by whatever it was ZOS did to the servers on June 25th. Latency and frame rate has never been this bad in the history of ESO, and the change that made it so happened on June 25th.
This is yet another way the players know performance issues are almost always server end when dealing with ZOS.
If nothing is done with this soon im just going to stop everything eso related...
No comments from Zos at all after me posting my tests really shows how much they care...
I keep getting booted from PCNA right after login. Maybe 3 or 4 minutes in, PCEU not an issue. I am also showing FPS in the 100s on both sides but seeing a slideshow versus gameplay
