The real reason for this is that areas do take a second to load Companions when they're first summoned. In most of the side rooms, the Companions are automatically desummoned, so when you get back to the main Archive you need to give them as second to summon properly. If you pull before they're ready, well...
That's what you get when you wake your friend up in the middle of a nap.6 -

M1SHAAN✭✭✭I've had this happen before when I realized halfway through a stage that I had forgotten to unsummon my banker, and then did so in the middle of combat. I didn't expect it to auto-summon my companion because I was in combat and you can't usually summon them while in combat, but the game was perfectly happy to summon them, just not their equipment. I felt so bad that I'd put them in that situation! Though it definitely had Skyrim-farmer-NPC-drops-everything-to-run-up-to-a-dragon-and-punch-it-in-the-face energy 3 -

✭✭✭✭✭From the times I see this happen with my companions, I think it is specifically if you engage the boss fight too quickly after leaving the portal from a side objective. The game spawns the companion back in, but doesn't do so entirely with gear and skills loaded out until the combat ends.1 -
