Mist form: is it considered a teleport ability to proc Crusader or Unleashed Terror set?

As per title can you explain to me if it procs on those kind of setes? Or maybe just the Blood Mist morph. Thank you!

Vaqual✭✭✭✭Crusader and Unleashed Terror both require the skill to do direct damage (first sentence of the tooltip), thus they do not proc from mist form.1
