I cannot get the Multiplanar devastator achievement

Hi,
I have been entering dozens of oblivion portals and destroyed the antecedent catalyst inside them, but according to the quest journal I have entered only 5 oblivion portals.
Even yesterday I entered a portal, killed the antecedent, but when I checked it still said "5".
So I cannot get the "portal punisher" achievement or the "Multiplanar devastator" achievement. I cannot find anywhere mentioned a bug.
Does anybody know if there are some special portals I need to use and that I might be missing? Should I use a different portal in a different area each time?

Are you going in from Blackwood or from the Deadlands?

Both take you to the "Atoll of Immolation," but only the ones sourced from the Blackwood zone will count for the achievement. Unfortunately, those do not have a chance to drop the Annihilarch motifs, so there likely aren't many people running them.
