With regard to #3, the developer for the add-on Furniture Catalogue would likely know, as he regularly datamines the ZOS database and updates the catalogue used by the add-on. I recall reading somewhere, perhaps in Patch Notes or in that add-on's online Log, that a few furnishing recipes which were Normal and Fine are no longer available in the game, and a few others which are Superior and Epic have been added. 1. They are all available in the game. There are no unique plans stored in the envelopes. (The separately purchasable gold plans are unique to Rolls/Faustina however)
2. The ones corresponding to the newest zones will have some plans that are worth buying them for (you'll make more profit on your vouchers), while there will be other more common plans. It's an RNG game.
3. No. AFAIK, they contain the entire drop loot table for those particular plans, so unless they retroactively add plans in, but I don't believe they've done so.
