It's my understanding that Major and minor vitality despite some language to the contrary actually work additively with healing taken set bonuses alone. So healing taken = Vitality and set piece bonuses that say "4% healing taken" only.

Healing received is comprised of Quick Recovery CP, the argonian and orc passives, the heavy armor passive, Sanctuary's 12% healing received buff, etc. Essentially, every healing buff that doesn't say healing done, vitality, or healing taken. I think a better way to think of it is as "other healing".

You also forgot the powered trait (9%), the 20% buff to healing done while near a keep, and the Ritual Mundus(10-15%).

You forgot the Dragonknight:Draconic Power class passive

Burning Heart II

Increases healing received by 12% while a Draconic ability is active.

The vast majority of tanks - this is a tank set - are DK so this is quite common.
