Healing Received and Healing Done are simply how much healing players get and give when standard healing skills that heal yourself or friendly players are used, excluding anything that is considered Healing Taken. Think of these as bi-directional terms for when players are healing each other with standard healing skills like those from the Restoration Staff or Restoring Light skill lines.
To the best of my knowledge anyone and everyone applies to these two types of healing boosts. So, if you have high healing done then your heals will be more potent and therefore heal everyone including yourself for more. Conversely, if you have high healing received then any heals you receive from anyone including yourself will be more potent and heal you for more.
Healing Taken is way more specific. The consensus of the community seems to be that Healing Taken is a special form of healing that you personally initiate by using a skill which steals from an enemy. Abilities like the Dragonknight Inhale or Nightblade Strife skills are perfect examples. If you have high healing taken, then you will heal yourself with qualifying abilities for more.
