Discrepencies between ingame tooltips and patch notes

As checking ingame the NB skill relentless focus only lists 40 per stack for the weapon/spell damage yet patch notes state 60 (which was week 1 of PTS but since then has undergone nerfs in later weeks)
Sacrificial Blast Bones as of Update 45 listed it buffing class abilities and DoTs, on live now it specifies necromancer abilities however this is not mentioned at all in patch notes
And finally for fatecarver it now does direct damage again this is something that was added throughout PTS cycle and yet no mention of the direct damage change in the live patch notes.
It really makes me wonder why patch notes get posted that are almost a 1:1 copy of week one PTS instead of the actual changes affecting the live server
At least the slimecraw nerf was mentioned this time.
Also ontop of that apparently a bunch of skill styles were added and no mention whatsoever in the Patch notes
