Combat Prayer not healing?

I main heal, but I noticed on multiple occasions that my combat prayer doesn't actually seem to be hitting anyone other than myself even though it very well should be.
One of my prog groups is progging vSE HMs and I'm playing group heal on Warden. I suspected something was wrong in a few instances before (in other trials as well) where it didn't seem like I was healing as much as I should be for the number of casts/uptimes I had, but lost all suspicion when the tomb healer for Yaseyla got caught in ice so I went to heal her out and, standing directly in front of her, combat prayer did nothing.
I know that sometimes terrain can cause heal targeting to be funny, but it didn't seem like anything was in the way, and even with target prioritization based on who entered the instance when, it healed the other person in the tomb directly behind her but not her (there was no one else in the cone it could have been hitting instead).
I was unfortunately not recording at the time, but was able to recreate the problem on my arcanist using the healing crystal ultimate and trying to heal it with a variety of different skills including combat prayer (energy orb, radiating regen, illustrious healing, the arc heal beam...) everything hit it except combat prayer healed literally nothing.
While this test does make me feel somewhat vindicated that it's not me and there actually *is* something wrong, it's also super frustrating when what *should* be my main spammable isn't reliably striking. To be clear, it is hitting sometimes, just not consistently and reliably. I don't really have any idea what could be causing the issue specifically, which is even worse since it's not clear how to avoid the problem as a result.
